Submissions after December 1, 2025 will be reimbursed in the new year. NOTES on Apparel Reimbursement. >>>attire may be reimbursed for NEW staff, or to reimburse clothing of staff whose previously covered attire is in need of being replaced (track suit is greater than two years old, or staff jacket greater than five years) and/or to be replaced as attire is damaged >>>Bench staff as named below will be covered for the following items ONLY, as listed. >>>Head Coach: Warm Up Suit or Winter Jacket >>>Assistant Coaches and On-ice staff; Warm Up Suit >>>Trainers: Winter Jacket or Vest NOTE: Only complete this form once your certification is complete,, and for apparel, once your order has been delivered. This form must be used when requesting reimbursement for Bench Staff certifications and apparel purchases (not covered by your competitive team).
